Website Information

Privacy and Accessibility

This page explains the intended handling of general website inquiries and Divinity's accessibility approach. It does not describe clinical records or replace any separate agreement governing services.

General inquiry information

The public contact form may collect a name, organization or facility, role, email address, optional telephone number, facility type, primary need, message, and consent to be contacted. Divinity uses this information to respond to the inquiry, understand the facility's general needs, and determine whether its services may be a fit.

Do not submit protected health information

Do not include resident names, dates of birth, diagnoses, records, or other protected health information in the public contact form. The form is for general business inquiries and is not monitored for emergencies.

Data minimization, access, and retention

Divinity intends to collect only the information needed for a general business inquiry, limit administrative access, and maintain a documented process for retaining and deleting contact-form submissions. Information submitted through the form is stored through the website's form service. Questions or requests concerning submitted inquiry information may be sent through the contact page without including resident information.

Analytics

Website analytics will be configured only after Divinity approves the privacy approach. If analytics are used, this notice should be updated to identify the categories of information collected and how that information is used.

Accessibility

Divinity is working toward WCAG 2.2 Level AA and aims to provide an experience that supports keyboard navigation, visible focus states, logical heading order, sufficient color contrast, readable form labels and instructions, assistive-technology-friendly feedback, and reduced-motion preferences.

If you encounter an accessibility barrier, contact Divinity through the contact page and describe the page and the problem. Do not include resident-identifying or protected health information.

Emergency notice

Divinity does not provide emergency or crisis-response services through this website. If someone is in immediate danger, contact 911 or the appropriate local emergency service.
